The Permanent Supportive Housing Program (PSHP) provides permanent housing and supportive services to individuals and families experiencing homelessness to ensure housing stabilization, maximum levels of self-sufficiency and an overall better quality of life. The first phase of this program is to assess individuals and families experiencing homelessness who are living on the streets, in shelters and other institutions. The second phase is to place individuals and families into long-term housing. The third phase is to provide effective case management to ensure that individuals and families are connected to needed support services and achieve the highest degree of stabilization and self-sufficiency possible.
Application: Individuals and families in the District of Columbia experiencing homelessness with a disabling condition who has either been continuously homeless for a year or more or has had at least four episodes of homelessness in the past three years are eligible to apply for the PSHP program.

To apply for the PSH program a vulnerability assessment survey must be completed at any District sponsored shelter facility or by a homeless service provider.
