Application: Call for more information. Applicants must be a District of Columbia resident, chronically homeless, a Veteran deemed eligible for the VASH Voucher by the Department of Veteran Affairs and considered vulnerable according to a vulnerability assessment survey. Only veterans may apply and be selected for the Veteran’s Affairs Supportive Housing Program. A vulnerability assessment survey must be completed by a community human/social services worker/provider and forwarded by mail to: Homeless Services Program Office 64 New York Avenue, NE, 5th Floor Washington, DC 20002 or fax form to (202) 442-6355.
