Transition Services

Organization: DC Center for Independent Living

DCCIL assists individuals with significant disabilities who live in nursing homes and other institutions to transition to community-based residences as well as assist in establishing community-based supports and services, provides assistance to individuals with significant disabilities who are at risk of entering institutions so that the individuals may remain in the community and facilitates the transition of youth who are individuals with significant disabilities.
